Participant Release and Waiver of Liability

•                Acknowledgment of Spiritual and Religious Nature
 I understand and affirm that Soul Full Bloom Fellowship is a spiritual and religious organization. Its ceremonies and sacrament offerings are conducted in accordance with sincerely held spiritual beliefs rooted in personal communion with the Divine through the sacred use of psychedelic and entheogenic sacraments.

These sacraments are offered solely within the context of spiritual practice, personal transformation, and ceremonial reverence. I acknowledge that my participation is voluntary and guided by my own spiritual calling.

•                Legal and Regulatory Disclaimer
 I understand that Soul Full Bloom Fellowship is not a licensed medical provider, therapist, or healthcare organization. I acknowledge that the facilitators are not offering diagnosis, medical treatment, therapy, or psychological advice.

Soul Full Bloom Fellowship operates under the First Amendment of the U.S. Constitution, the Religious Freedom Restoration Act (RFRA), and relevant Colorado constitutional protections, affirming its rights to sacramental use of psychedelic and entheogenic substances within a sincere spiritual framework.

I further acknowledge that:

•                Many psychedelic substances remain classified as Schedule I substances under federal law, although Colorado law (Natural Medicine Health Act, 2022) decriminalizes certain personal uses of natural psychedelic medicines.

•                Soul Full Bloom Fellowship is not a licensed healing center under the Colorado Natural Medicine Program and does not operate under state-sanctioned facilitator credentials.

•                My participation is not protected by medical, therapeutic, or insurance standards, and I accept full personal and spiritual responsibility.

•                Voluntary Participation & Assumption of Risk
 I am voluntarily choosing to participate in spiritual practices that may include sacramental use of psychedelic and entheogenic substances. I understand these substances can produce powerful physical, psychological, and emotional effects.

I affirm that I am:

•                In a stable mental, emotional, and physical condition to participate.

•                Not currently experiencing or recently experienced psychosis, mania, or other severe psychiatric conditions unless I have discussed this fully with a qualified spiritual guide or health provider.

•                Not taking any contraindicated medications (e.g., SSRIs, antipsychotics, MAOIs) that could pose a safety risk with the sacraments, or have disclosed these to the appropriate Fellowship guide.

I assume full responsibility for any risks, including but not limited to:

•                Psychological distress, challenging emotions, or trauma resurfacing

•                Physical effects, including nausea, dizziness, or adverse reactions

•                Legal consequences arising from my own conduct related to sacraments

•                Personal Responsibility & Integration
 I understand that:

•                The Fellowship and its guides offer spiritual support, integration practices, and community care, not clinical therapy or treatment.

•                I am responsible for my own spiritual integration and aftercare.

•                I have the right to decline or stop participation at any time.

•                Confidentiality & Mutual Respect
 I agree to uphold the confidentiality of all participants, respecting their experiences, privacy, and spiritual processes. I will not disclose the names, stories, or identities of others without their explicit permission.

•                Waiver and Release of Liability
 I, for myself, my heirs, executors, and assigns, hereby release and hold harmless Soul Full Bloom Fellowship, its officers, clergy, guides, volunteers, and agents from any and all liability for injury, death, loss, legal risk, or damages arising from or related to my participation in any Fellowship activity.

I understand that no guarantees or promises of healing or outcomes have been made. I accept full responsibility for any and all consequences, known or unknown, that may result from my participation.

•                Governing Law and Dispute Resolution
 This agreement shall be governed by the laws of the State of Colorado. In the event of any dispute, both parties agree to resolve the matter through binding arbitration in Colorado, with each party bearing their own costs unless otherwise decided by the arbitrator.

•                Informed Consent & Entire Agreement
 By signing below, I acknowledge that:

•                I have read and fully understand this release.

•                I am entering into this agreement freely, voluntarily, and without coercion.

This document represents the entire understanding between myself and Soul Full Bloom Fellowship concerning these matters.
